If Medicine is at School:  

School policy requires written permission to keep both prescription and over-the-counter meds on campus. Please note that even cough drops are considered medication and can not be sent to school without written permission. 

All medications need to be in it's original packaging or medicine bottle. Prescription medications do require a physician signature. Please see forms below if you would like to have medication on hand here at the school for your child. 

These forms need to be filled out once per every school year.
